Log on to the Federal Government's website www.nslds.ed.gov to see the colleges and universities you have attended and received Federal Aid (Pell and/or Direct Loans) from during the 2021-2022, 2022-2023, 2023-2024, and 2024-2025 academic years. Click on Financial Aid Review and complete the requested information on this form. Dates Attended (not Name of Schools Attended Number of Academic Credits terms).

Attach a TRANSCRIPT (official or unofficial) or grade reports for each school listed.
NOTE: the committee may request additional documentation.
