Federal Title IV regulations require students to be in Satisfactory Academic Progress (SAP) to remain eligible for financial aid. Our records indicate that your Pace of Progression is below the minimum requirement of 67% or your GPA is still below 2.0. To remain eligible for financial aid, you must submit an academic plan to our office. Please refer to the financial aid email sent to you for the number of credits that are required for you included in this Academic Plan. You may need to meet with your Academic Advisor or one of our Navigators to determine what courses you will be taking each term to complete this form. Attach an additional page if more than 4 terms are needed. [for example: You are notified at the end of Spring term that you need 15 credits on your academic plan; you are registered for 12 credits Fall term, you will need to fill out your classes/credits for your Fall term classes AND estimate what your Winter term classes will be.]
The financial aid office will review your plan at the end of each term to ensure that your pace of progression and/or GPA is improving. If your either goes down, you withdraw or fail any class you will be placed on financial aid Suspension. By signing this form, you understand the terms of this Academic Plan.
